DUI Science: Are Gastric Bypass Patients More Susceptible to a DUI?

According to the results of a new study in the February issue of the Journal of the American College of Surgeons, the dramatic changes that occur as a result of gastric bypass surgery can cause some people to overindulge when using alcohol thereby increasing their risk for a DUI. As cited at by Science Daily (linked HERE):

Studies have shown that gastric bypass patients often find it difficult adjusting to physical and psychological changes after the procedure. An increased risk of depression, alcoholism, and other substance abuse issues for this patient population led researchers to take a more in-depth look at how these patients metabolize alcohol after the procedure.  The results of this unique demonstration of alcohol metabolism changes in gastric bypass patients showed that patients who underwent a Roux-en-Y gastric bypass (RYGB) operation had considerably higher breath alcohol content (BAC) and took significantly more time to return to a sober state after drinking, compared with BAC levels tested prior to having their procedure.

The studies demonstrated that peak BAC after drinking five ounces of alcohol were greatly increased after the operation. “BAC was 0.024 percent at pre-operation and 0.059 percent (p = 0.0003) at three months. Tested again at six months post-operation, the patients’ BAC was 0.088 percent (p = 0.0008) which is more than the legal driving limit of .08 percent.” Id. Obviously, if a person who has had gastric bypass (also called bariatric surgery) decides to drink they should take their body changes into account prior to finding themselves in a position where they drive an automobile.

    I’ve done a lot of research on this subject. I had the surgery in 2000 and my husband had the surgery in 2001. He is now an alcoholic with multiple DUI’s. The problems isn’t solely the increased potency alcohol now has on the gastric bypass patient, but what drives them to return to alcohol again and again, after that first experience. I would recommend reading the recent study by Dr. Magdalena Plecka Ostlund of the Karolinska Institutet who just released her findings showing that patients that had the Lap-Band type procedure weren’t having these types of problems like the bypass patients are (typically Roux-en-Y the most common procedure). That may explain the alcoholism, but it wouldn’t explain the significant rise in substance abuse, depression and suicide attempt. My research has led me to believe that the long term malabsorption of nutrients is changing the brain function and setting up those neural pathways, predisposing people towards anxiety and depression which they attempt to self-medicate, leading to this extraordinary rise in this problem among gastric bypass patients. It may be difficult to prove at this time, but more and more health care professionals are becoming aware of this and realizing it’s not this “addiction transfer” psychological phenomena they first attributed to this. They are also documenting many other problems associated with nutrient deficiency associated with the Roux-en-Y because it bypassed the duodenum and upper jejunum. There is another procedure called the Duodenal-Switch which doesn’t bypass the entire duodenum allowing for the absorption of the critical nutrients the Roux-en-Y procedure doesn’t. The DS procedure bypasses a longer piece of intestine, but it is the piece further along the intestinal tract, and with regards to nutrient absorption, this is critical. There is speculation that the DS patients are not having these addiction/depression problems but without the data, it cannot be documented as yet.
